After I got married, I decided to change my last name. Only problem was that I had no idea how to go about it, and there isn’t a straightforward list on the internet. So, here is a little help for you newlyweds!
Have a copy of the marriage certificate showing new last name.
Apply for new Social Security Card by filling form SS-5 (one page).
Bring completed SS-5, proof of ID, and DHS work authorization if applicable to your local social security office.
Update your Driver’s License with new name (fees will apply)
Inform every one of your name change: work, bank, credit cards, insurance, utilities place (s), Facebook, Passport, Student ID, Library Card, Gym Membership Card. There you have it. No need to be intimidated by the process :)
